Long term condition reviews 

We offer yearly review and medication check for patients with the following conditions:

  • After bariatric surgery 
  • Asthma 
  • Atrial fibrillation 
  • Bronchiectasis
  • Chronic kidney disease 
  • Chronic obstructive airways disease (COPD, Emphysema)
  • Coeliac disease 
  • Coronary heart disease
  • Diabetes – type 1, type 2 and gestational diabetes
  • Epilepsy 
  • Gout 
  • Heart failure 
  • High blood pressure (hypertension)

  • Inherited raised cholesterol 
  • Mental health conditions 
  • Metabolic-dysfunction associated steatotic liver disease (MASLD, previously known as NAFLD or ‘fatty liver’)
  • Monoclonal gammopathy of uncertain significance (MGUS)
  • Over- and under-active thyroid disorders 
  • Parathyroid hormone problems 
  • Polycystic ovarian syndrome 
  • Pre-diabetes 
  • Rheumatoid arthritis
  • Stroke and TIA (‘mini-stroke’)

For all these conditions you will be sent a reminder of your review just before or at the beginning of your birth month. This will be sent by your preferred contact method, usually a text message or email with a link to book an appointment.

Initial Appointment

You can book this appointment online using the booking link that has been sent to you, or by calling reception if you are unable to use the online booking system. 

This appointment will be with a healthcare assistant and will focus on your overall health and lifestyle. You will be asked any questions which were not included in the pre- appointment questionnaires that are relevant to your condition. 

In the appointment, depending on your condition we may:

  • Take a blood test
  • Check your weight
  • Ask for your waist circumference
  • Check your blood pressure
  • Check your feet
  • Do an ECG

Things to bring to the appointment:

  • If you have Hypertension (raised blood pressure), diabetes, or chronic kidney disease (CKD), please bring an early morning urine sample to this review.
    • Containers can be collected in advance from reception – these should be ‘yellow top’ bottles.
  • Blood pressure diary
    • Measuring blood pressure at home and taking an average of the home readings is now a preferred way of measuring blood pressure than taking a single reading in the surgery. 
    • If you have not received a blood pressure recording text message or recording sheet please bring a 4 day record with at least 2 readings per day with you to your yearly review if possible. 
  • Please bring any questions or concerns you have about your condition or treatment to your review and we can ensure these are recorded for the team to discuss with you. 

Follow Up

After the initial appointment the nurse will review the results of your tests along with information from your questionnaires and, if needed, may invite you for a further appointment to discuss your treatment plan. 

If your results show that your condition appears to be under control you will receive a summary of your care plan. 

One of the doctors or pharmacists will then review your medicines and update your recall for the next year.  This may sometimes need a further discussion with you before completion. 

All long term conditions benefit from a healthy lifestyle which will often reduce progression of the condition and can reduce the number of medications you need to take.
